$20.19/hr   The Reentry Navigator will provide in-reach and transitional and reentry navigation services recovery services to individuals re-entering into the community from incarceration. Navigators will work with program participants to develop and implement treatment plans that will service and support their transition back into the community, provide supported referrals to address participants’ needs, and to assist program participant to obtain their identified goals.

Hourly rate: $42.00/hour   The Registered Nurse (RN) participates in the coordination of medical, behavioral health/addiction treatment services and other supports in accordance with the person-centered treatment plan to manage or co-manage complex behavioral health cases.  As a member of the multidisciplinary team, the RN provides consultation, education, and support to team members and provider network regarding co-occurring medical issues related to behavioral health/substance abuse treatment and management.
