The Reintegration Clinician will work with the mental health team at Worcester County Jail and House of Corrections to focus on providing enhanced reintegration services. They will provide counseling and clinical case management to recently incarcerated individuals to provide continuity of care between incarceration and the community. They will provide outreach to high risk individuals and encourage engagement in services.
