The Reintegration Clinician will work with the mental health team at Worcester County Jail and House of Corrections to focus on providing enhanced reintegration services. They will provide counseling and clinical case management to recently incarcerated individuals to provide continuity of care between incarceration and the community. They will provide outreach to high risk individuals and encourage engagement in services.

• Develop a clinical reentry program focusing on improving transition back into the community for currently incarcerated and/or recently released individuals.
• Work both within the jail and at the re-entry center to provide individual and group therapy to individuals releasing from incarceration.
• Facilitate referrals to community providers for recently released inmates if appropriate.
• Work collaboratively with jail staff and community providers to enhance continuity of care.
• Provide outreach and follow up to recently incarcerated individuals to identify needs and provide support and resources.
• Maintain necessary documentation to include, assessments, file notes, and treatment plans

• Master’s degree in Social Work, Counseling Psychology, or related counseling field.
• Current independent license or license eligible (requiring supervision which can be provided) with the Massachusetts Board of Registration in Social Work or Mental Health Counseling.
• Must have working knowledge of current version of the American Psychiatric Association’s Diagnostic and Statistical Manual of Mental Disorders.
• Experience or interest in working with a forensic population.
• Demonstrated understanding and competence in working with culturally diverse populations.
• Ability to collaborate and effectively work within a multidisciplinary team
