Salary Range: $49,920-$52,000

The Transition Coordinator is responsible for the referral and placement process and assisting the Program Director to monitor overall program compliance and supervision of Shared Living Coordinators. In addition, this role will carry a reduced case load to coordinate, provide, and supervise high quality supports to the individuals and caregivers served by Shared Living program.

Responsibilities

  1. Collaborate with the Shared Living Referral Team; Program Director, Clinician and Recruiter to oversee and conduct intake assessments and ensure appropriate placements are made.
  2. Provide support with the recruitment process for Shared Living Caregivers.
  3. Participate in interviews for potential caregivers in collaboration with the Recruiter.
  4. Ensure the referral database is updated and the Referral Intake and Placement (RIP) Form for each referral is completed.
  5. Oversee and finalize the placement checklist and open an electronic record folder for each new admission.
  6. Communicate regularly with applicable stakeholders to give updates on the referral process.
  7. Coordinate admission process of new individual’s; gather intake paperwork, create required binders, complete assessments, documentation, and initial home visits during the first quarter.
  8. Assist the Program Director with internal audits to ensure overall program compliance.
  9. Carry a reduced caseload and implement all aspects of the program according to Advocates policies and procedures and state licensing and certification regulations.
  10. Act as program liaison to local DDS Area Office, Survey & Certification, and other Department personnel as delineated by Program Director.
  11. Act as liaison with other agencies involved in providing services to the people we support such as day programs.
  12. Develop positive relationships with individuals, families, and monitor/assess placements minimally on a monthly basis.
  13. Support and oversee participating caregivers.  Train caregivers and families in all facets of their responsibilities.
  14. Support the development and growth of all individuals served in the program.
  15. Provide crisis intervention and 24 hour on call responsibility.

Qualifications

  1. BA/BS in social work or related field and 4-6 years’ experience in Human Services field.
  2. Sales & recruitment background preferred.
  3. Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  4. Experience with database entry, maintenance and report production.
  5. Experience with Microsoft Word, Microsoft Excel, Microsoft Outlook, Adobe Acrobat, and Adobe PageMaker.
  6. High energy level, superior interpersonal skills, and ability to function in a team atmosphere.
  7. Must possess excellent organizational skills and ability to multi-task.
  8. Ability to maintain professional, pleasant, and helpful demeanor at all times.
  9. Ability to work independently and must be flexible with hours.
  10. Ability to read and speak English.
  11. Must hold a valid drivers’ license.  Must have access to an operational and insured vehicle and be willing to use it to transport people supported.
