The Certified Community Behavioral Health Clinic (CCBHC) Assertive Community Treatment (ACT) Outreach Worker provides intensive face-to-face and telephonic outreach with CCBHC ACT participants diagnosed with serious mental illness who have needs that have not been well met by traditional approaches to delivery services.

Responsibilities

  • Provide face-to-face and telephonic support to CCBHC participants receiving ACT services to keep them engaged in services, and support them in implementing their ACT Integrated Treatment Plan.
  • Meet with participants in the community to provide psychiatric rehabilitation interventions to support their continued tenure in the community.
  • Receive training and implement assigned CCBHC participants receiving ACT services through Evidence-Based Practices including ACT and Motivational Interviewing.
  • Liaison to shelter, housing, behavioral and physical health providers, and other community providers to collaborate in locating and engaging CCBHC participants receiving ACT in community resources, as needed.
  • Maintain professional, pleasant and helpful demeanor at all times.
  • Ensure all paperwork, reports, and trainings are up to date and completed within the expected timelines.
  • Provide on-call coverage for emergency crisis intervention and consultation.
  • Attend and actively participate in all supervisions and staff meetings.
  • Attend trainings as assigned; maintain necessary certifications and licenses.

Qualifications

  • Bachelor’s degree in social work, mental health counseling, or related field.
  • Minimum of one-year experience working with people with SMI in community-based settings strongly preferred. 
  • Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Ability to communicate effectively verbally and in writing in the primary language of the program to which assigned
  • Strong computer knowledge.
  • Ability to thrive in a fast-paced, team-oriented environment and as well as work independently.
  • Strong organizational skills and ability to multi-task.
  • Outstanding reasoning and problem-solving abilities.
  • High energy level and superior interpersonal skills.
  • Must hold a valid driver’s license, have access to an operational, insured vehicle, and be willing to use it to transport clients.
