    The Licensed Practical Nurse provides nursing services as defined in the LPN scope of practice and under the direction of the Director of Health Services.

    Responsibilities

    • Assist individuals with all aspects of medical management including physicals, referrals, prescriptions, appointments and treatments.
    • Coordinate with individuals, family members, physicians, other medical providers, and site team regarding all medical care and changes in status, treatment or medication.
    • Provide nursing case management to identified individuals at risk.
    • Oversee the flow of medical documentation through EMR systems.
    • Participate at the Interdisciplinary Team meetings.
    • Monitor and evaluate individuals care and generate ideas for quality improvement.
    • Provide trainings to site teams to include: vital signs, health protocols, person specific medical trainings, and DPH approved trainings.
    • Implement, oversee, and supervise medication management in accordance with agency and funder regulations.
    • Provide emergency first aid and direct staff in the case of a medical crisis.
    • Promote and maintain a safe environment.
    • Complete all documentation in a timely manner to ensure the availability of accurate information in accordance with established standards.
    • Participate in staff meetings, trainings, service planning meetings, resident and family meetings.
    • Attend trainings as assigned; maintain necessary certifications (CPR, MA LPN)
    • Attends and actively participates in supervision and staff meetings
    • Adhere to all principles related to the Advocates Way
    • Ensure that individuals we support are treated with dignity and respect in accordance with Advocates’ Human Rights policy and Intentional Care Standards
    • Ensure that individuals we support are treated with dignity and respect in accordance with the Brain Injury Services Division Human Rights Policy.
    • Attend and actively participate in staff meetings and in-service training, as required.
    • Perform all duties in accordance with the agency’s policies and procedures.

    Qualifications

    • Graduate of an approved nursing school with current licensure in the Commonwealth of Massachusetts as a Licensed Practical Nurse with 1-year experience in a community healthcare setting caring for individuals with I/DD.
    • Fluency in American Sign Language (ASL).
    • Strong oral and written communication skills with individuals, families, funding sources, consultants, the community at large, and the program and administrative staff.
    • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
    • High energy level, superior interpersonal skills and ability to function in a team atmosphere
    • Must be able to perform each essential duty satisfactorily.
    • Strong computer knowledge.
    • Must hold a valid drivers’ license.  Must have access to an operational and insured vehicle and be willing to use it to transport individuals.
