    Responsibilities

    Responsibilities include, but are not limited to:

    • Oversee high quality placements that support individuals and caregivers served by Shared Living Program.
    • Develop positive relationships with individuals, caregivers, families and monitor/assess placements minimally on a monthly basis.
    • Train caregivers and families in all facets of their responsibilities
    • To act as program liaison to local DDS Area Office, Survey & Certification, and other Department personnel as delineated by Program Director.
    • Collaborate with other agencies involved in providing services to the people we support such as vocational & day programs.
    • Ensure that the program meets and complies with all applicable DDS and other state agency regulations: overseeing compliance of caregivers with mandated trainings.
    • All responsibilities as identified in DDS regulations re "placement services” Process individual referrals and aspects of the coordination of the admission process.
    • Collaborate with the Caregiver Recruiter and participate in recruitment, selection, and evaluation of shared living families.

    Qualifications

    • BS/BA in related field with 1-2 years of experience working with adults/children with special needs. (Applicants with prior human services or teaching experience strongly preferred.)
    • Must hold a valid drivers’ license.  Must have access to an operational and insured vehicle. 
    • Ability to partially work from home is an option after six months of employment. Anticipated client visit schedule 3-4 days per week. Flexibility to set own schedule around client needs. 
    • Ability to communicate effectively verbally and in writing and able to use good judgment.
    • Must be able to perform each essential duty satisfactorily.
    • Sensitivity to the needs of the individuals we support.
    • High energy level, superior interpersonal skills and ability to function in a team atmosphere.
