    This residential program supports Transitional Aged Youth between the ages of 18-25 who are receiving DMH services. 

     

    We are looking for energetic, creative, and upbeat individuals who love to work with young adults and a team of equally energetic direct care peers!   Do you love to play a game of pick-up basketball?  Xbox?  Do you love to see a young adult delight in their first paycheck and help them learn how to budget it? This could be the program for you!

    Responsibilities

    • Provide ongoing support, guidance, role modeling and supervision to clients in a residential setting, referring other service providers when needed.
    • Participate in the development of the Individualized Action Plans (IAP) and attend other treatment meetings, including weekly staff meetings.
    • Collaboratively develop and document Program Comprehensive Assessments (CAs) and reviews, outlining rehabilitative goals and interventions, which reflect program participant preference and meet Department of Mental Health and Agency standards.
    • Communicate program participant's progress, mental status, and any changes in daily CBFS notes.
    • Address and work toward identified goals and skills development with the clients through appropriate coaching and intervention techniques.
    • Identify and address clients’ community integration barriers and difficulties.
    • Monitor and document medication administration.
    • Ensure a clean, safe and home-like environment for clients.
    • Provide crisis intervention and access emergency services as needed.

    Qualifications

    • Bachelor’s Degree; or High School Diploma or equivalent degree and one year of related experience.
    • Excellent interpersonal, judgement and coaching skills.
    • Strong written and verbal communication skills.
    • Ability to utilize basic computer applications.
    • Desire to thrive in a fast-paced, client-centered, team-oriented environment.
    • Excellent written and verbal communication.
    • Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients.
    • Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
