    Our In-Home services professionals offer one-on-one support to the individual within the comfort of their own home and community.  This unique model will allow you to develop a meaningful and trusting relationship that assures guidance, inclusion, and empowerment to the individual, while offering continuous and thoughtful support with daily activities.  By exercising your motivational efforts and creative strengths, you will encourage the individual’s growth and happiness.

     

    Our Brain Injury Services team is actively seeking female In-Home Support Staff to partner with a vibrant, active woman who lives in Lexington.  In addition to providing assistance with day-to-day and recreational activities that stimulate the mind and body, you will work together to establish meaningful goals and provide the encouragement needed to strengthen her growth and independence.

    Responsibilities

    • Provide ongoing support, guidance and role modeling for the individual.  Assist individual in self-advocacy, decision-making and empowerment.
    • Collaborate with the individual, family members and program management, as part of the multidisciplinary team in providing support to the individual.
    • Participate in planning and implementation of activities and daily routine for the individual.
    • Support the individual in her home and encourage her to become an active member of their community by actively seeking out integrated activities, opportunities for community participation and membership.
    • Assist the individual to identify and develop person-centered goals and plans to address any concerns.
    • Provide transportation, and lift and transfer as needed.
    • Prepare summaries of interaction and objective behavioral observations of clients at end of shift.
    • Report significant behavioral and psychiatric health-related concerns.
    • Perform physical intervention in the event of a client crisis.

    Qualifications

    • High School diploma or GED. Experience with the program population is preferred.
    • Excellent written and verbal communication.
    • High energy level and ability to function in a team atmosphere.
    • Ability to communicate effectively both verbally and in writing and ability to use good judgment.
    • Ability to work in a team environment as well as independently.
    • Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients.
    • Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
