The Director of Population Health and Quality Management is responsible for oversight of care management team functions including delivered Community Partner (CP) supports, population health management, quality management, and service improvement; and for developing, managing, implementing, and documenting the Quality Improvement Program.

The Behavioral Health Partners of MetroWest (BHPMW) provides collaborative care coordination and wellness goal support to individuals with behavioral health and/or substance use needs, based on the recent transformation of Mass Health’s service delivery model.  The partnership is built on a collaboration among five community-based provider agencies: Advocates, Family Continuity, South Middlesex Opportunity Council (SMOC), Spectrum Health Systems, and Wayside Youth and Family Support Network, throughout MetroWest, Central and Northern Massachusetts. 




  • Receive data from MassHealth, MCOs, ACOs and internally; collaborate with data analytics to analyze data and identify actionable items and deploy providers of CP supports to act on data.
  • Establish mechanisms to identify, communicate, and act on care events and care transitions.
  • Liaise with Program Managers (e.g., Care Management leads at consortium entities and affiliated partners) and Quality Management managers to ensure implementation of all aspects of the Quality Improvement program.
  • Implement quality improvement activities. Identify best practices to achieve good outcomes on all required metrics and integrate into staff training and protocols.
  • In collaboration with the Program Director organize, plan, and convene the Quality Management Committee and ensure its responsibilities are completed and documented.
  • Ensure documentation compliance and quality and adherence to all requirements related to the delivery of BH CP supports and activities.
  • Oversee and collaborate with the BHPMW Data Analyst and Compliance Officer.
  • Collaborate with the BHPMW Triage/Resource Coordinator to manage Enrollee assignments efficiently.
  • Work with data analyst to oversee outcomes metrics.
  • Work with quality managers at the consortium entities and affiliated partner to conduct chart review.
  • Demonstrate a proactive commitment to maintaining effective communication with staff, Enrollees, Care Team members and other collaterals, and other internal and external stakeholders and customers.  Promote strong working relationships and excellent customer service.
  • Provide supervision and professional development opportunities to all team members.
  • Strictly adhere to HIPPA and confidentiality policies at all time, including when responding to inquiries for Enrollee information.
  • Oversee participation in experience of care survey to be administered by an external vendor.
  • Manage DSRIP accountability score and quality score, in collaboration with data analytics.
  • Oversees staff orientation and training, both initial and ongoing.
  • Oversees enrollee protections and representation including grievances, enrollee rights, information and accessibility requirements as well as critical incidents.



  • Master’s Degree in social work or a related field.  Minimum of two years of care management experience, and two years of supervisory experience, any of which may overlap. 
  • Massachusetts independent practice licensure preferred; license-eligible candidates are also encouraged to apply!
  • Training in care management and integration of medical and behavioral health services preferred.
  • Experience in program development, collaboration, and contract management with external customers preferred.
  • Strong communication and follow through skills with a focus on collaboration and customer service.
  • Computer skills, including proficiency with Microsoft Office Suite programs, with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies.
  • Must hold a valid driver's license and have access to an operational and insured vehicle.
  • Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
