The Case Manager will have primary responsibility for assigned cases and for the implementation of those Day Habilitation Service Plans (DHSP).  The Case Manager is responsible for providing developmental skills training for assigned individuals.  The Case Manager should be proactive and must focus on individual advocacy, empowerment, and community integration.

Responsibilities

  • Develop and implement a comprehensive written therapeutic service plan (DHSP) and to complete all documentation which supports such plan.
  • Develop, implement, and monitor creative, therapeutic goals with individuals.
  • Complete daily data, monthly progress notes and quarterly reviews.
  • Provide skills training and support to the individuals as identified in the DHSP, to include: training in self-care, self-preservation, receptive and expressive communication, self-direction, activities of daily living, hygiene, grooming, and first aid.
  • Provide a safe environment for the individuals.
  • Assist individuals in self-advocacy, decision making and empowerment.
  • Foster community integration and provide support to individuals to actualize this goal.
  • Provide case management for individuals in the program as identified by supervisor, advocate for all individuals.
  • Report significant issues (health, behavioral, psychiatric) to the appropriate supervisor.  Document issues/incidents according to agency guidelines.

Qualifications

  • Bachelor’s Degree; or two years' experience working with individuals with developmental disabilities.
  • Strong interpersonal skills and ability to use good judgement.
  • Excellent written and verbal communication skills.
  • Basic computer knowledge.
  • Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients.
  • Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
