Advocates is seeking enthusiastic, driven RNs to offer medical support and care coordination to those in need.


The Registered Nurse (RN) in the One Care program assists members in aspects of medical health concerns, including advocating with physicians and other health care providers.  The RN provides care management, medical teaching and training to members.  The RN actively participates in key activities related to the Minimum Data Set (MDS) assessment, completion and submission process.  As a member of a multidisciplinary team the RN provides consultation and support to other members of the team.

  • Complete the Minimum Data Set (MDS) in the member’s home or other preferred setting.
  • Document assessments and care plans in electronic medical record.
  • Maintain documentation, administrative, quality standards and fiscal duties required by each contract.
  • Work collaboratively and effectively with care management, including individual receiving services, medical team and other behavioral health providers to provide behavioral health care management services.
  • Participate in team meetings to ensure effective communication among disciplines involved in client’s care.
  • Follow the client across the continuum of care to ensure care coordination.
  • Partner with the client, their care team providers and supports to ensure that care plan and safety plan are implemented as developed, and to make as needed adjustments.
  • Monitor client’s progress in treatment and assist the clinical team with evaluating the need for continued services.
  • Identify community resources to develop natural supports for client.


  • Bachelor’s Degree in Nursing and a current, state of MA, registration in nursing and one year of experience in a community health care setting.
  • Must have strong verbal and written communication skills with members, funding sources, consultants, the community at large, and internal staff.
  • High energy level, superior interpersonal skills and ability to function in a team atmosphere.
  • Demonstrated knowledge base and judgment necessary for independent clinical decision making.
  • Basic computer knowledge.
  • Knowledge regarding psychiatric rehab and understanding of recovery model.
  • Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients.
  • Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
