Starting rate: $62,400 - $67, 600

 

The Shared Living Program Director leads the administration and financial coordination of new referrals and supports Shared Living Coordinators through supervision, guidance, and coaching to ensure effective delivery service.

Responsibilities

  • Ensure implementation of philosophy of service delivery for Shared Living and implement the Advocates Way Philosophy and Intentional Care Standards.
  • Assure high quality supports by providing direction and supervision in all aspects of the program. Perform all responsibilities as identified in DDS regulations re "Placement Services”.
  • Participate in on call rotation of 24 hours/day, 7 days/week.
  • Supports programmatic needs in absence of Shared Living Director of Operations
  • Act as program liaison to DDS Area Office/s, Office of Quality Enhancement for Licensing and Certification, CARF and other Department personnel as delineated by Shared Living Director of Operations.
  • Act as liaison with other agencies involved in providing services to the individuals, we support such as day programs.
  • Maintain communication internally and external with staff, families/guardians, shared living caregivers vendors, funding source(s) and other stakeholders.
  • Attend, arrange, and conduct meetings for Shared Living members with the funding source, clinicians, therapist, day program staff and family/guardians to resolve any issues or concerns that may arise, as needed.
  • Responsible for supervision and evaluation of assigned Shared Living Coordinators including core trainings, development trainings and program specific trainings to perform duties adequately.
  • Ensure quality management throughout the program in conjunction with agency QM department. Conduct internal audits to ensure overall program compliance with all applicable DDS and funder regulations.
  • Ensure health and safety of individuals and staff through compliance with agency policies, OSHA, and other safety standards.
  • Support the development and growth of all individuals served in the program. Develop and monitor creative and interesting community centered opportunities for all individuals. Ensure a person-centered approach.
  • Ensure that the needs of the individuals are addressed through verbal, written and email communication and staff receive the information pertinent to the individuals needs.
  • Assist individuals with obtaining all services and entitlements for which they qualify.
  • Collaborate and prepare for clinical case reviews with the Shared Living multidisciplinary team.
  • Lead/co-lead team meetings for the purpose of education, skill growth, program planning, and improving program functioning.
  • Collaborate with the Shared Living Referral Team; Shared Living Program Director of Operations, Clinician and Recruiter to oversee and conduct intake assessments and ensure appropriate placements are made.
  • Complete initial assessments for new individuals.
  • Responsible for the budgetary completion for incoming individuals
  • Oversee training of shared living caregivers in all facets of their responsibilities.
  • Oversee and implement shared living policies & procedures.
  • Ensure that individuals are treated with dignity and respect in accordance with Advocates’ Human Rights policies
  • Attend and actively participates in assigned trainings; maintain necessary certifications.
  • Participate in statewide shared living round table meetings and the yearly shared living conference.
  • Perform all duties in accordance with the agency’s policies and procedures.

Qualifications

  • BA/BS degree in human services or equivalent and 6-8 years experience working with disabled/elderly adults in community setting. Experience with administrative functions including supervision of staff, hiring, staff development and evaluation.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Sensitivity to the needs of the population we support
  • High energy level, superior interpersonal skills and ability to function in a team atmosphere
  • Ability to communicate effectively verbally and in writing and ability to use good judgment.
  • Strong analytical, numerical, and reasoning abilities.
  • Must be able to perform each essential duty satisfactorily.
  • Working knowledge of Microsoft Office Suite.
  • Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport individuals.
