Overview

Starting Salary: $19.50/hour

Schedule: Monday through Friday 8am-4pm

 

The Day Services Case Manager supports individuals in a day program setting by helping design and implement engaging, person-centered activities that promote independence, skill development, and community involvement.

 

You will work as part of an interdisciplinary team to develop activity schedules, provide direct skills training, and support individuals in achieving their personal goals. This role also includes documentation, progress notes, and collaboration with families and team members to ensure high-quality, coordinated supports.

Minimum Education Required

High School Diploma/GED

Responsibilities

  • Assist in developing curriculum and structured programming to promote independence and growth
  • Design and implement activity schedules and therapeutic/recreational programming
  • Provide direct skills training and individualized support
  • Develop creative, innovative programming to enhance engagement and empowerment
  • Collect and maintain daily data, progress notes, and semi-annual reports
  • Provide direct care and support with activities of daily living as needed
  • Maintain active communication with families, funders, consultants, and team members
  • Participate in team meetings, supervision, and required trainings
  • Maintain certifications including First Aid, CPR, Safety Care, and HR/DPPC
  • Ensure all services align with Advocates’ Human Rights Policy and organizational values

Qualifications

  • Associate’s degree in a related field and 1 year of experience or high school diploma or equivalent and 2+ years of relevant human services experience in lieu of a degree
  • Strong written communication and documentation skills
  • Ability to multitask in a fast-paced, changing environment
  • Strong computer skills required
  • Ability to work independently and as part of a team
  • Commitment to person-centered, respectful support practices
  • Ability to communicate effectively in English
  • Valid driver’s license, reliable vehicle, and willingness to transport individuals
  • Experience working with culturally diverse populations strongly preferred
