Starting Rate: $46.50/hour

The Residential Programs Nurse provides direct nursing oversight and management of medical and care components in accordance with Advocates and funder requirements.  The Residential Program Nurse provides training, supervision, and oversight for both residents and staff in all aspects of medical and medication management.  They will collaborate with the Nursing Director, Residential Program Directors and nursing team in the implementation and follow-through of the individualized service plans and documentation in accordance with the mission and regulations of the program.

Responsibilities include, but are not limited to:

• 1. Take a lead role in assessing potential program participants at his or her residential site; identify the program participant’s needs and functional abilities.
  2. Coordinate assessment for new durable medical equipment needs. Provide oversite and ensure on- going equipment checks and regularly scheduled maintenance.

  1. Participate initially and at least annually in a team assessment with the applicant, family member / guardian, and Residential Program Director as to whether the applicant can manage their personal care services.
  2. Complete the annual assessments related to the program participant’s support needs.
  3. Ensure Physician’s Authorization for each GAFC resident twice annually per Medicaid site policies (if applicable).
  4. Prior to an applicant’s admission to the program, complete a Medication Assessment and develop an Emergency sheet, Medication List, nursing care plan, guidelines, protocols, individualized ADL sheet and coordinate with the Residential Program Director to complete a risk plan.
  5. Develop an individualized Emergency Contact sheet, which includes family, guardian, physicians’ (PCP and specialists), clinic/hospital contact information, Social Security Number, MassHealth ID number, date of birth, gender, guardianship, health care proxy, marital status and code status, and program start date for each new resident prior to his/her start at the program.
  6. Assist residents with all aspects of medical management including physicals, referrals, prescriptions, appointments, and treatments.
  7. Coordinate with residents, family members, physicians, other medical providers, and site team regarding all medical care and changes in status, treatment, or medication.
  8. Implement, oversee, and supervise medication management and best practices in accordance with agency policy relating to MAP in a manner consistent with funder regulations.
  9. Provide supervision and oversight in Supported Living Program (SLP) programs and nursing team in coordination with the Nursing Director.
  10. Provide and document person-centered training to direct care staff as part of the nursing team. Develop and implement trainings as part of the nursing team as needed.
  11. Promote and maintain a safe environment:
  12. Complete all documentation in a timely manner to ensure the availability of accurate information in accordance with established standards.
  13. Monitor and evaluate resident care and generate ideas for quality improvement.
  14. Remain available on call 24 hours per day, 7 days per week.  Schedule and notify off days with Nursing Director to ensure on call coverage.
  15. Actively participate in staff meetings, trainings, service planning meetings, care coordination meetings, resident, and family meetings.
  16. Attend trainings as assigned; maintains necessary certifications (CPR, MA Registration in Nursing).
  17. Ensure that individuals we support are treated with dignity and respect in accordance with the Brain Injury Services Division’s Human Rights Policy.
  18. Perform all duties in accordance with the agency’s policies and procedures.

• 1. Current license to practice in Massachusetts as a Registered Nurse required. Current certification in CPR / Basic Cardiac Life Support required.  Graduate of an Accredited School of Nursing with at least 3 years’ clinical experience required.  CRRN preferred.  Experience in management, supervision, and bachelor’s degree or equivalent preferred.  Experience with individuals with brain injuries in a community setting preferred.
  2. Must be able to perform each essential function satisfactorily.
  3. Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  4. Ability to correlate clinical data with resident's medical and nursing care plan.
  5. Ability to communicate effectively both verbally and in writing and ability to use good judgment.
  6. Ability to set priorities and handle multiple simultaneous tasks, organizing, prioritizing, planning, sequencing and completing tasks independently.
  7. Basic computer knowledge.
  8. Ability to understand, speak, read and write fluent English.
  9. Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport individuals.
  10. Ability to operate agency vehicles
