The licensed Triage Clinician will implement a system for triaging requests for service, in collaboration with the team, to determine the most appropriate response to individuals in crisis and assist individuals by helping them identify and access resources for treatment. The Triage Clinician will follow a system of triaging requests for services via walk in, phone, and other avenues. The Triage clinician will collaborate with Person Served, MCI, Outpatient, and other service lines to allow for immediate access to care. This includes cross training in multiple service lines assessments. This includes flexibility to complete assessments in multiple services lines to complete timeliness and access to care. Responsibilities

  • Maintain a triage system for all requests for service made to CBHC, including Same Day Access (SDA), Urgent Care, and Mobile Crisis Intervention (MCI).
  • Collaborate with MCI and OP to ensure appropriate regulatory timeliness metrics are met
  • Complete Outpatient and MCI Evaluations based off the need of the individual
  • Collaborate with individual and OP team to ensure direct scheduling occurs for individual and meets regulatory timeliness standards.
  • Provide phone and telehealth coverage for triage as needed.
  • Determine the most appropriate response to clients in crisis in collaboration with the team.
  • Provide/complete referrals (internal and external) as needed for new members.
  • Provide information on community resources, treatment and providers, and referral services to individuals.
  • Collaborate with all programs within the CBHC as needed.
  • Assist in the billing of crisis intervention services.
  • Maintain necessary documentation, statistics, and clinical records in accordance with program requirements.
  • Ensure follow up appointments are directly scheduled for individual.
  • Assist in maintenance and management of electronic health record database.
  • Develop a working knowledge of the internal billing procedures and managed care requirements.
  • Complete multi disciplinary team chart reviews. Review all paperwork, recordkeeping and electronic Health records. Ensure standards are met and timelines are followed and clinical work is reflected
  • Ensure charts are ordered and upheld to standards. Ensures all charts have PSQ scales, faxed PCP forms, CANS, and labs etc as required.
  •  Assist in maintenance and management of electronic health record database.
  • Develop a working knowledge of the internal billing procedures and managed care requirements.
  • Remain alert at all times throughout the scheduled shift.
  • Adhere to all principles related to the Advocates Way.
  • Attend and actively participates in supervision and team meetings.
  • Perform all duties in accordance with the agency’s policies and procedures.
  • Strictly follow all agency Performance Standards.

 

Qualifications

 

  • Master’s Degree in social work, counseling, or related field.
  • Massachusetts independent practice licensure in Mental Health Counseling or Social Work.
  • Two years of mental health or emergency service experience preferred.
  • Demonstrated understanding of and competence in serving culturally diverse populations.
  • Ability to develop and maintain satisfactory working relationships with employees and providers.
  • Ability to communicate effectively verbally and in writing.
  • Ability to read and speak English.
  • Sensitivity to the needs of the population.
  • Commitment to Advocates values and mission.
  • High energy level, superior interpersonal skills, and ability to function in a team atmosphere.
  • Must have valid driver’s license and access to auto.
