The Licensed Practical Nurse – LPN participates in the coordination of medical, behavioral health/addiction treatment services and other supports in accordance with the person-centered treatment plan and works collaboratively with the Behavioral Health Community Partner (BHCP) team at their respective agency to manage or co-manage complex behavioral health cases. As a member of the multidisciplinary team, the LPN support to team members and provider network regarding co-occurring medical issues related to behavioral health/substance abuse treatment and management.
