The Senior Health Care Coordinator is a Registered Nurse overseeing all health care services provided to the individuals within the day services program.  This position will work closely with an interdisciplinary team to ensure the health and wellbeing of all individuals through routine assessment, hands on care and training.  They will oversee and lead staff and other nurses within the day services setting.

Responsibilities

  • Monitor all health and wellbeing for all individuals with ongoing correspondence with primary care physicians/nurse practitioners, community members, etc. 
  • Participate in the admission and discharge procedures for day services participants, including assessment in conjunction with the Director of Day Habilitation Nursing.
  • Provide supervision, training, and support to nursing staff regarding job responsibilities, program development and services.
  • Monitor recommendations in conjunction with the comprehensive written therapeutic service plan (DHSP) process through the interdisciplinary team. 
  • Provide detailed quarterly and semiannual reviews on all aspects of individuals’ day habilitation programming.
  • Provide daily nursing notes and summaries as needed in accordance with Agency/division expectations and deadlines. 
  • Evaluate all individuals in day habilitation annually providing a detailed plan of care.
  • Review, maintain and update medical information in each file.
  • Work alongside the Program Managers to ensure all confidential files are complete, up-to-date and meet all applicable regulations.
  • Oversee and administer prescribed medication as necessary, including oral medication and injections.
  • Provide daily nursing care to individuals (ie. colostomy care, G-tube, catheter, oxygen treatments, etc).
  • Assist with community inclusion for those requiring nursing oversight and care.
  • Provide structured opportunities for groups and individuals to learn healthy living and wellness systems, as well as regarding health/medical information.
  • Provide emergency first aid and provide support, direction and oversight to staff in the case of a medical crisis.
  • Train all staff regarding medical policies/protocols as necessary and ensure the implementation and follow-through.
  • Develop effective working partnerships and maintain open lines of communication with all residential providers, caregivers, families, and DDS/MRC.
  • Provide and set a positive team atmosphere
  • Train and guide staff on understanding of regulations and requirements for individuals within day habilitation services.
  • Provide training to staff within all Developmental and Brain injury day, employment, and community services.
  • Assure compliance with all Masshealth and state regulations as they pertain to Day services.

 

Qualifications

  • Must possess a current, state of MA, registration in nursing.
  • Two years of experience in a community health care setting.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Must have excellent written and verbal communication skills
  • Must possess strong computer knowledge and strong analytical, numerical and reasoning abilities,
  • Ability to execute a variety of decision-making models and to multi-task with frequently changing priorities.
  • Must hold a valid drivers’ license.  Must have access to an operational and insured vehicle and be willing to use it to transport individuals supported.
