The Care Coordinator provides information, referrals, and care coordination to people seeking behavioral health (BH), social services, and primary healthcare needs.  Works in collaboration with the Project Director and other team members in the development of systems, services, and internal and external relationships.  BHPMW and its MetroWest Care Connection hub serves as a single point of entry into the full continuum of BH and social services operated by the legal partnership of Advocates, South Middlesex Opportunity Council (SMOC) and Wayside Youth and Family Support Network (Wayside).

Responsibilities

  • Answer live calls, respond to online requests for services; assess needs and triage people to needed services; coordinate care with persons served, their families, PCPs, and service providers internal and external to the BHPMW partner agencies and follow up to ensure people are well connected to the services they need and want.
  • Ensure that services are delivered in accordance with a welcoming, recovery oriented, trauma informed, culturally sensitive environment and service delivery approach.  Ensure that clients, staff, and collaterals are treated with dignity and respect.
  • Develop and maintain deep knowledge of services offered by the partner and affiliate agencies and community resources. 
  • Develop close working relationships with key staff at partner agency programs and community resources while collaborating to ensure coordination of care.
  • Broker timely access to needed services within the partner and affiliate agencies and other resources, ensuring access standards are met.
  • Demonstrate a proactive commitment to maintaining effective communication with co-workers and customers to facilitate and promote strong working relationships and excellent customer service.
  • In collaboration with Project Director and team members, participate in community education and market services of the BHPMW both internally and externally as appropriate.
  • Attend and actively participate in all supervision and staff meetings.
  • Attend trainings as assigned; maintain necessary certifications and licenses.
  • Ensure all required documentation and reports are submitted on time in accordance with BHPMW and department due dates.
  • Ensure that clients are treated with dignity and respect in accordance with Advocates Human Rights Policy and similar policies of the other partner agencies.
  • Adhere to all principles related to the Advocates Way and the values and philosophies of the other partner agencies.
  • Perform all duties in accordance with the agency’s policies and procedures.

Qualifications

  • Bilingual (Spanish) preferred 
  • Bachelor’s or Associate degree in social work, behavioral health, human services, healthcare or a related field or High School/GED with significant relevant experience in human services.
  • Intake and/or care coordination experience strongly preferred. 
  • Demonstrated knowledge base and judgment necessary for independent decision making.
  • Training in care coordination and integration of medical and behavioral health services.
  • Experience developing and managing internal and external relationships.
  • Broad knowledge of MetroWest communities, populations and service systems.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Ability to read and write English and communicate effectively in the primary language of the program to which he/she is assigned.
  • High energy level, flexibility, superior interpersonal skills, and ability to function in a multidisciplinary team atmosphere while also working independently.
  • Strong skills in the areas of organization, follow through, collaboration, customer service, problem solving, and negotiation.
  • Commitment to BHPMW and partner agencies’ values, missions, and goals.
  • Computer skills, including Word, Excel and Outlook, ability to learn new systems, and experience working with data.
  • Ability to perform each essential duty satisfactorily.
  • Must hold a valid driver's license and access to an operational and insured vehicle.
  • This is a part time position, 24 hours per week
