In Home Therapy Clinician to work in Waltham, MA. Conducts comprehensive, home-based behavioral health assessment inclusive of the CANS (Child and Adolescent Needs and Strengths standardized assessment tool); develops and coordinates treatment planning; provides intensive family therapy; identifies community resources and develops natural supports for youth and parent/guardian/caregiver; provides complete care coordination: attends necessary meetings, to include, IEP meetings, hospital discharge meetings, Care Planning Team (CPT) meetings for members enrolled in ICC; provides collateral contacts for case coordination (phone and face to face); collaborates with care coordinator (for youth enrolled in ICC services) and/or behavioral service providers for treatment planning; provides referrals and linkage to appropriate supports and services; coordinates and communicates to collaterals all aftercare planning; provides coaching in support of decision making in both crisis and non-crisis situations; provides skills training for youth and parent/caregiver/guardian; provides phone crisis response during working hours and participates in on-call coverage rotation; provides member/service recipient transportation as needed in connection with service delivery; conducts member outreach as needed to engage members and families; participates in Risk management/safety planning; completes all required documentation in a timely manner; attends and actively participates in supervision and staff meetings; develops and maintains working relationships with CSA (Community Service Agencies as part of Mass Health’s managed care contractors) and community partners. Requires: Masters’ degree in Counseling, Social work, Psychology or related field, Certification to administer CANS-MA version, and 2 years relevant work experience.
