The Medical Appointment Assistant is responsible for supporting a Senior Program Manager with all medical appointment related duties and ensuring the smooth operation of two group homes.

Minimum Education Required

Responsibilities

  1. Schedule, transport and attend all individuals’ medical, dental, psychiatric and any other necessary appointments in collaboration with the Senior Program Manager and/or guardians.
  2. Complete pre and post documentation of every appointment and ensure post appointment update is communicated and shared internally to the residential leadership including nursing and clinical and externally to other identified providers (DDS, day programs, and family/guardians).
  3. Monitor and maintain the individual’s medication supply in collaboration with the Senior Program Manager and the residential teams.
  4. Update, transcribe and ensure the accuracy of all medication orders following Medication Administration Program procedures within the electronic health care record.
  5. Ensure all individuals’ health insurance is current.
  6. Assist individuals in Self-Advocacy, decision-making, and empowerment in regard to any health-related needs and support self-medication goals and objectives.
  7. Attend all staff meetings.
  8. Prepare medical appointment summaries for Senior Program Manger to be shared at DDS quarterly update meetings.
  9. Maintain continuous written and verbal communication and report all significant issues to the appropriate persons, as well as document issues/incidents according to agency guidelines.
  10. Attend trainings as assigned; always maintain necessary certifications (First Aid, CPR, Safety Care, MAP, PBS, HR/DPPC).
  11. Ensure that individuals we support are treated with dignity and respect in accordance with Advocates’ Human Rights policy.
  12. Attends and actively participates in supervision.
  13. Perform all duties in accordance with the agency’s policies and procedures.

Qualifications

  1. High School Diploma or GED and two years related experience.
  2. Must be able to perform each essential duty satisfactorily.
  3. Ability to communicate effectively verbally and in writing and the ability to use good judgment.
  4. Sensitivity to the needs of the population.
  5. Must have basic computer knowledge.
  6. Must be highly organized and possess the ability to multitask in high energy atmosphere.
  7. Must have superior interpersonal skills and ability to function in a team atmosphere.
  8. Must hold a valid drivers’ license.  Must have access to an operational and insured vehicle and be willing to use it to transport individuals.
