The Care Manager provides focused leadership, supervision and support for the care coordination staff.  This role coordinates behavioral health and substance use disorder care management services in accordance with the individualized care plan, and works collaboratively with the clinical team to co-manage supports and services needed for individuals with complex behavioral health needs.  As a member of the multidisciplinary team, the Care Manager provides consultation, education, and support to team members and provider network regarding behavioral health and substance abuse treatment and management.

Responsibilities

  • Hire, train, supervise and evaluate care coordination staff.  Maintain consistent communication with staff and conduct regularly scheduled meetings with staff.
  • Provider support to outreach, engage and assist clients in identifying their health and wellness, and recovery goals and developing their individualized care plans.
  • Utilize effective, dignified, empowering and creative engagement strategies to ensure clients are at the center and lead in their care team.
  • Provide linkages to medical care, behavioral health services, substance use disorder programs and community resources and supports.
  • Provide assistance with navigating the healthcare system and provide support and assistance with transitions in care.
  • Collaborate with existing providers and other collaterals and delivers services in accordance with client’s plan of care.  Facilitate communication and information exchange across the continuum of providers.
  • Coordinate all aspects of service delivery with team members, including ensuring access and securing authorization, as indicated.
  • Participate in team meetings to ensure effective communication among all disciplines involved in client’s care.
  • Follow the client across the continuum of care for the purposes are care coordination.
  • Partner with the client, their care team providers and supports to ensure that care plan and safety plan are implemented as developed, and to make as needed adjustments.
  • Monitor client’s progress in treatment and assists the clinical team with evaluating the need for continued services.
  • Identify community resources and develop natural supports for client.
  • Provide on-call coverage, as needed.

Qualifications

  • Masters degree with first level license (LICSW, LCSW, LMFT, LMHC, LMFT licensed psychologies, or LADAC I with minimum of 2 years of experience working in a community-based mental health, substance use and/or physical health setting.
  • Minimum of 2 years’ supervisory experience, preferred.
  • Demonstrated knowledge base and judgment necessary for independent clinical decision making.
  • Minimum of 3 years professional and clinical experience in the areas of mental health, counseling, and substance use.
  • Strong skills in the areas of communication, follow through, collaboration, and customer service.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Excellent interpersonal skills and demonstrated ability to interact professionally with culturally and educationally diverse staff and patients.
  • Creativity, flexibility, sound judgment, and the ability to take initiative.
  • Excellent time management and organizational skills.
  • Strong Computer skills, including proficiency in contemporary Windows operating systems and Windows office suites with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies.
  • Strong communication and writing skills.
  • Demonstrated ability to work as an effective team member and leader in a complex and fast-paced environment.
  • Knowledge regarding psychiatric rehab and understanding of recovery model.
  • Must be able to perform each essential duty satisfactorily.
  • Must hold a valid drivers’ license.  Must have access to an operational and insured vehicle and be willing to use it to transport members.
