The Documentation Specialist is responsible for overseeing the development and implementation of the clinical record to ensure compliance with paperwork timelines, guidelines, and federal and state agency documentation standards.

1. Ensure the people we serve have treatment plans that meet DDS, DMH, Mass Health and/or MRC standards and timelines.
2. Review all paperwork, recordkeeping, and electronic health records.  Ensure documentation standards are met, timelines are followed, and clinical work is reflected accurately.  Provide oversight to ensure all charts have necessary annual documents and relevant updates.
3. Meet regularly with the Program Directors, Program Managers, and Clinical and Nursing team members to review all paperwork. 
4. Provide initial and ongoing support and training to all program staff in documentation standards, daily notes, treatment plans, electronic health care records, and record keeping requirements.
5. Develop and present trainings or workshops on best practices and on-going updates related to the management of required electronic documentation.
6. Provide supervision to all staff responsible for writing ACES (Advocates Community Electronic System) related documentation, HCSIS (Home & Community Services Information System) documentation, and the Individual Support Plans (ISP).
7. Maintain tracking and notification system to facilitate timely documentation completion.
8. Conduct ongoing training with staff to promote high quality paperwork standards.
9. Collaborate with the Quality Management Consultants to ensure charts pass licensing and certification reviews from Department of Developmental Services and CARF (Commission on Accreditation of Rehabilitation Facilities); to include ensuring records are compliant with these entities.
10. Attend and actively participate in all supervision, services leadership meetings, and staff meetings as required.
11. Attend trainings as assigned; maintain necessary certifications as required.
12. Ensure that individuals we support are treated with dignity and respect in accordance with Advocates’ Human Rights policy.

1. High School Diploma or equivalent and minimum of two years of clinical experience.  1 year experience with DDS, IDD/BI waiver residential experience.
2. Must hold a valid driver's license and access to an operational and insured vehicle to travel between sites.
3. Excellent communication skills with the ability to read and write English and communicate effectively in the primary language of the program to which they are assigned.
4. Excellent computer skills.  Ability to use an Electronic Health Record.
5. Ability to thrive in an independent role while offering and receiving team support.
6. Intuitive, client-centered, supportive approach.

7. Ability to set priorities, work under time constraints and manage multiple projects simultaneously.
8. Strong attention to detail.
9. Must be able to perform each essential duty satisfactorily.
