*Starting Rate: $21/hour*

 

The Senior Case Manager is responsible for ensuring that the highest quality support to be afforded to individuals with autism spectrum disorder who receive Community Based Day Services. The Senior Case Manager assists the Director with the larger focus on guiding the day-to-day operations of the program.

They are responsible for overseeing those enrolled in Autism CBDS, designing an environment that offers job readiness; community integration, skills training, social skill building and recreational activities.

Responsibilities

  • Collaborate with the Team Leader to provide continuous quality improvement of individualized programs and services through the use of both internal and community resources. 
  • Assist team members with the development of program curriculum designed to promote individual growth and self determination. 
  • Develop and implement comprehensive written therapeutic service plans/individual support plans and implementation process for all assigned consumers.
  • Convey changes in consumer goals to the larger internal support team.
  • Maintain member records as required including timely and thorough completion of necessary program assessments, progress reports and other required documentation.
  • Compile review summaries for therapeutic service plans/individual support plans.

 

Qualifications

  • Associate’s Degree in related field and 1 year of related experience or in lieu of a degree a minimum of 3 years of related experience.
  • Must be able to perform each essential duty satisfactorily.
  • Must have ability to communicate effectively in writing.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Must have ability to read English and communicate effectively in the primary language of the program to which he/she is assigned.
  • Must be able to multi-task with frequently changing priorities.
  • Must possess basic computer knowledge.
  • Must have a demonstrated ability to work independently and as part of a larger resource team.
  • Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
