The Advocates Day Habilitation Program in Ashland, MA is looking for a Licensed Practical Nurse (LPN) to join their team of nurses. The Day Habilitation Program serves adults ages 22 and over with developmental disabilities to engage in therapeutic and recreational programming. The LPN provides daily nursing services including medication administration, g-tube feeding administration, first aid, and more. This is a full time position with daytime weekday hours- no weekends or major holidays. $5,000 sign on bonus and generous benefits offered. New graduates welcome.

Responsibilities

Responsibilities include, but are not limited to:

  • Administer prescribed medication as necessary.
  • Consult with Team Leaders, Case Managers, Skills Instructors, Aids and other essential individuals on a day-to-day basis to share any pertinent medical information.
  • Monitor recommendations in conjunction with the comprehensive written therapeutic service plan (DHSP/ISP) process and the interdisciplinary team.  Provide daily nursing notes and monthly summaries as necessary.  Review, maintain and update medical information in each file.
  • Promote community inclusion and self-advocacy.
  • Collaborate with and provide consultation to Team Leaders, Case Managers and Skills Instructors to develop and implement DHSP/ISP curricula in pertinent areas.
  • Provide emergency first aid and direct staff in the case of a medical crisis.
  • Attend and actively participate in staff meetings and in-service training, as required.
  • Attend trainings as assigned; maintain necessary certifications

Qualifications

  • Graduate of an approved nursing school with current licensure in the Commonwealth of Massachusetts as a Licensed Practical Nurse. 1 year experience in a community healthcare setting preferred.
  • Strong oral and written communication skills with individuals, families, funding sources, consultants, the community at large, and the program and administrative staff.
  • Must be able to perform each essential duty satisfactorily.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Basic computer knowledge.
  • Must hold a valid drivers’ license.
