Overview

The Mobile Community Support Person will facilitate accessible, holistic, and comprehensive care for the individuals supported. The Integrated Care Coordinator will primarily focus on the integration of behavioral and physical healthcare management for individuals served by Advocates and Reliant.

Responsibilities

  • Work collaboratively and effectively with individuals receiving services, medical team, and behavioral health providers to provide integrated care management services within the community.
  •  Collaborate with existing providers and other collaterals and coordinate services.
  • Foster transitions between providers to create a seamless collaborative care approach.
  •  Coordinate integrated team meetings and actively participate to ensure effective communication among the team.
  •  Complete paperwork in Electronic Health Records (EHR), including release forms and referral requests.
  • Completes authorization paperwork as needed to demonstrate medical necessity of services.
  •  Participate in team meetings to ensure effective communication among all disciplines involved in patient’s care.
  •  Follow the patient across the continuum of care for the purposes of care coordination.
  •  Partner with the patient, his/her care team providers and supports to ensure that treatment plan and safety plan are implemented as developed and make as needed adjustments.
  •  Monitor patient’s progress in treatment and assist the clinical team with evaluating the need for continued services.
  •  Identify community resources and develop natural supports for patient.
  •  Complete all required documentation in a timely manner.
  • Provide outreach services to patients, as needed.
  •  Maintain up to date medical records including quarterly reviews, termination summaries and progress notes.

Qualifications

 

  • BSW degree, BA in related field from an accredited college/university or 1+ years of related experience
  •  One + years of direct care and/or care coordination experience or related experience.
  • Ability to use an Electronic Health Record to document medically necessary clinical services.
  •  Must be able to perform each essential duty satisfactorily.
  • Ability to communicate effectively verbally and in writing.
  •  Bilingual/trilingual (Spanish/Portuguese) preferred; candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  •  Commitment to Advocates’ values and mission.
  •  Must have valid driver’s license and access to automobile
