Salary: $65,000

The Hybrid Clinical Intake Specialist works as part of a rapid response team; providing brief treatment and wrap around services to individuals and families, who have had a member recently seen by the emergency room or hospital.  The Hybrid Clinical Intake Specialist makes initial contact with the member, schedules an intake for soon after discharge, completes intake and supporting documents, identifies need areas and provides a warm transition to the team of clinicians and care coordinators that will provide follow up care; connecting the individual to needed resources and providing bridge supports while providers are being identified.

Responsibilities

  • Conduct behavioral assessments on children and adults and write care plans which address their individual needs.
  • Utilize effective, dignified, empowering and creative engagement strategies to ensure individuals are at the center and lead in their Care Team.
  • Coordinate all aspects of service delivery with team members, including ensuring access and securing authorization.
  • Collaborate with existing providers and other collaterals and deliver services in accordance with individual’s plan of care.
  • Participate in team meetings to ensure effective communication among all disciplines involved in member’s care.
  • Follow the individual across the continuum of care for the purposes of care coordination.
  • Partner with the individual, their care team providers and supports to ensure that care plans and safety plans are implemented as developed. As needed, make adjustments
  • Monitor individual’s progress in treatment and assists the clinical team with evaluating the need for continued services.
  • Identify community resources and develop natural supports.
  • Complete all required documentation in a timely manner.
  • Attend and actively participate in supervision and staff meetings.
  • Provide on-call coverage, as needed.

Qualifications

  • Master’s degree in social work, mental health counseling or related field with minimum of 2 years experience working in a community-based and/or medical setting.
  • Demonstrated knowledge base and judgment necessary for independent clinical decision making.
  • Excellent verbal and written communications skills. Strong interpersonal and customer relations skills required.
  • Excellent organizational, time management and problem-solving skills.
  • Ability to openly address and acknowledge issues of substance use and mental illness.
  • Knowledge regarding psychiatric rehab and understanding of recovery model.
  • Must be able to perform each essential duty satisfactorily.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Ability to develop and maintain satisfactory working relationships with employees and providers.
  • Commitment to Advocates values and mission.
  • Must hold a valid drivers’ license.  Must have access to an operational and insured vehicle and be willing to use it to transport members.
