The Care Management Supervisor works in conjunction with the Integrated Care Management Director of Operations to provide focused leadership, supervision and support for the care coordination staff.  The Care Management Supervisor coordinates behavioral health and substance use disorder care management services in accordance with the individualized care planThey work collaboratively with the clinical team to manage or co-manage supports/services needed for individuals with complex behavioral health needsAs a member of the multidisciplinary team, the Care Management Supervisor provides consultation, education, and support to team members and provider network regarding behavioral health/substance abuse treatment and management.

Responsibilities

  • Hire, train, supervise and evaluate care coordination staffMaintain consistent communication with staff and conduct regularly scheduled meetings with staff. 
  • Monitor program documentation to ensure accuracy and timeliness. 
  • Support the Integrated Care Management Director of Operations in the management and administration of the assigned programs and monitor and improve productivity. 
  • Provide support to outreach, engage and assist clients in identifying their health/wellness and recovery goals and developing their individualized care plans. 
  • Utilize effective, dignified, empowering and creative engagement strategies to ensure clients are at the center and lead in their care team. 
  • Provide linkages to medical care, behavioral health services, substance use disorder programs and community resources and supports. 
  • Provide assistance with navigating the healthcare system and provide support/assistance with transitions in care. 
  • Collaborate with existing providers and other collaterals and delivers services in accordance with member’s plan of careFacilitate communication and information exchange across the continuum of providers. 
  • Coordinate all aspects of service delivery with team members, including ensuring access and securing authorization, as indicated. 
  • Participate in team meetings to ensure effective communication among all disciplines involved in member’s care. 
  • Follow the member across the continuum of care for the purposes are care coordination. 
  • Partner with the client, their care team providers and supports to ensure that care plan and safety plan are implemented as developed, and to make as needed adjustments. 
  • Monitor member’s progress in treatment and assists the clinical team with evaluating the need for continued services. 
  • Identify community resources and develop natural supports for client. 
  • Complete all required documentation in a timely manner meeting all Agency and division established deadlines. 
  • Attends and actively participates in supervision and staff meetings. 
  • Attend assigned trainings; maintain necessary certifications and licenses (CPR/First Aid, Safety Care, MAP). 

Qualifications

  • Master’s degree in social work, mental health counseling or related field with minimum of 2 years experience working with adults in a community-based and/or medical setting,  
  • One year of supervisory experience preferred. 
  • Demonstrated knowledge base and judgment necessary for independent clinical decision making. 
  • Excellent verbal and written communications skills. Strong interpersonal and customer relations skills required 
  • Strong computer knowledge 
  • Excellent organizational, time management and problem solving skills. 
  • Ability to openly address and acknowledge issues of substance use and mental illness. 
  • Knowledge regarding psychiatric rehab and understanding of recovery model. 
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations. 
  • Must hold a valid drivers’ licenseMust have access to an operational and insured vehicle and be willing to use it to transport members.
