The Registered Nurse/Medical Coordinator oversees all health care services provided to the individuals at the designated locations.

Additional Shift Details

Location: Worcester area – Grafton, Northbridge, Douglas, Medway, Holliston, and Franklin.

Hours: Monday – Friday 8am-4pm, hours have some flexibility.

Responsibilities

Seeking a RN with 1+ years of nursing experience needed for HMEA, an agency supporting individuals with Developmental Disabilities for a medical coordination/case management role. Some experience with IDD preferred but not required. Responsibilities include regular communication with Residential Managers and coordinating/attending appointments, procedures, hospitalizations, admissions, discharges, as well as providing training to staff and assessments. Travel is required and mileage is reimbursed. This position is eligible for benefits and the hours have some flexibility based on the needs of the programs and individuals we support. This position is Monday through Friday, no weekends, no holidays, and no mandated overtime.

Qualifications

  • Must possess a current, state of MA, registration in nursing.
  • One year of experience in a community health care setting.
  • Must be able to perform each essential duty satisfactorily.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Strong oral and written communication skills with individuals, families, funding sources, consultants, the community at large, and the program and administrative staff.
  • Must hold a valid drivers’ license.  Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
