Overview

Starting Rate: $19.50/hour

 

The Home & Community Support staff will support a young man within his home and within the community offering recreation and volunteer opportunity supports.  This 35-40 hour a week position will work alongside the individual to develop job readiness; therapeutic and recreational activities to fill the day.  This may include supporting the individual while volunteering at a food pantry, delivering meals to community members; exercising and outdoor activities; learning home and life skills.  A male candidate is preferred, as well as a valid drivers license and car.

Responsibilities

  • Collaborate with the Program Manager and Director to provide continuous quality improvement of individualized programs and services through the use of both internal and community resources. 
  • Ensure that individual’s input to their interests and preferences have been assessed and are assisted in achieving their personal goals.
  • Assist in the development of innovative program ideas and alternative therapeutic services that foster individual’s empowerment and program satisfaction.
  • Implement volunteerism and community activities to various places in the local community.
  • Develop relationships with volunteer partners and community establishments to enhance volunteer program.
  • Provide vocational skills training and support to individual.
  • Teach social skills for a community, work and program setting.
  • Responsible for the completion of daily data, monthly progress notes, semi annual reports and other necessary paperwork.
  • Communicate any participant needs to the Manager and Director.
  • Ensure continuous communication between staff, individual, families/caregivers, funding sources, and day services leadership team.
  • Ensure daily attendance/billing is complete and accurate.
  • Ensure that all individuals’ personal care needs are met.
  • Remain alert at all times throughout scheduled workday.
  • Ensure that individuals we support are treated with dignity and respect in accordance with Advocates’ Human Rights Policy.
  • Adhere to all principles related to the Advocates Way.
  • Attend trainings as assigned; maintain necessary certifications (First Aid, CPR, Safety Care, HR/DPPC).
  • Attends and actively participates in team meetings and supervision.

Qualifications

 

  • Associate’s Degree in related field and 1 year of related experience or in lieu of a degree a minimum of 3 years of related experience.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Basic computer knowledge.
  • Demonstrate strong oral and written skills (English proficiency) with individuals, families, funding sources, consultants, the community at large, program and administrative team.
  • Demonstrate an ability to work independently and as part of a larger resource team.
  • Proven ability to effectively supervise and mentor staff and training
  • Must be able to multi-task with frequently changing priorities.
  • Must hold a valid drivers’ license.  Must have access to an operational and insured vehicle and be willing to use it to transport individuals.
  • Must be able to perform each essential duty satisfactorily.
